This Grand Round session will focus on Thromboembolic Disorders in pregnancy. It will be led by Dr. Hillary Aheisibwe. He is an Associate Consultant OBGYN/Fertility Specialist at Mbarara Regional Referral Hospital- Department of Obstetrics & Gynaecology.
Overview
Thromboembolic disorders are among the leading causes of maternal morbidity and mortality, with pregnancy increasing the risk of thromboembolism 4–5 times due to hypercoagulability, venous stasis, and endothelial injury. The main manifestations are deep vein thrombosis, which commonly affects the left leg, and pulmonary embolism, the leading direct cause of maternal death. Risk factors include previous VTE, thrombophilias, obesity, advanced maternal age, immobility, cesarean delivery, and preeclampsia. Diagnosis relies on imaging such as compression Doppler for DVT and V/Q scan or CTPA for suspected PE, as D-dimer is not reliable in pregnancy.
Management is centered on anticoagulation with low molecular weight heparin, alongside risk assessment and prophylaxis to prevent recurrence during pregnancy and the postpartum period.
