Overview
Opportunistic infections remain a major cause of morbidity and mortality among patients with advanced HIV disease. Toxoplasma gondii is classically known for its cerebral involvement, but disseminated disease affecting other vital organs is rare and often fatal. Cardiac toxoplasmosis, in particular, is an uncommon but life-threatening manifestation that is seldom diagnosed before death.
In this grand round, we present a rare and fatal case of HIV-related toxoplasmosis with simultaneous cerebral and cardiac involvement, confirmed at autopsy. The case illustrates the diagnostic challenges, highlights the pathological spectrum of Toxoplasma gondii infection in immunocompromised patients, and emphasizes the need for vigilance in considering multi-organ disease in advanced HIV.
